ยา manidipine และยา amlodipine เป็นยารักษาโรคความดันโลหิตสูงในกลุ่ม calcium channel blockers (CCBs) ยากลุ่มนี้แบ่งออกเป็น 3 รุ่นตามการพัฒนาของยาและคุณสมบัติทาง pharmacokinetics และ pharmacodynamics ยา Manidipine เป็นยา CCBs รุ่นที่ 3 โดยไดรับการพัฒนามาจากยา Amlodipine ซึ่งเป็น CCBs รุ่นที่ 2 [1,2] calcium channel มี 6 subtypes ได้แก่ L T N P Q และ R

เปรียบเทียบยา amlodipine และยา manidipine ดังตารางที่แนบไว้

ผลของ calcium channel blockers (CCBs) ต่อ glomerular hemodynamic ประเมินจากความสมดุลระหว่างการลดลงของ afferent arteriole resistant และการลดระดับ systemic blood pressure ถึงแม้ว่ายากลุ่ม conventional CCBs เช่น amlodipine สามารถลดระดับความดันโลหิตแต่ผลในการทำให้เกิดความบกพร่องของ renal autoregulation ส่งผลให้เกิด glomerulosclerosis ได้ รวมถึงการเพิ่มขึ้นของ glomerular hyperfiltration และ glomerular pressure ที่ส่งผลให้เกิดภาวะ albuminuria และการดำเนินไปของโรคไตได้ ในทางตรงกันข้าม ยาในกลุ่ม novel calcium antagonists เช่น manidipine สามารถชะลอการเสื่อมของไตได้จากผลลด intraglomerular pressure และลดการขับ urinary albumin [2] อย่างไรก็ดี ตามคำแนะนำของ KDIGO guidelines 2021 Clinical Practice Guideline for the Management of Blood Pressure in Chronic Kidney Disease และแนวทางการรักษาโรคความดันโลหิตสูงในเวชปฏิบัติทั่วไป พ.ศ. 2562 ยังคงแนะนำให้ใช้ยาในกลุ่ม renin-angiotensin system blockers กลุ่ม ACEIs (angiotensin-converting enzyme inhibitors) หรือ ARBs (angiotensin-receptor blockers) ในผู้ป่วยความดันโลหิตสูงที่มีภาวะมีโปรตีนในปัสสาวะ (proteinuria) ร่วม[10,11 ]ทั้งนี้เมื่อเปรียบเทียบประสิทธิภาพในการลดภาวะ proteinuria ของ manidipine และ enalapril ซึ่งเป็นยาในกลุ่ม ACEIs ยา enarapril สามารถลด Systolic Blood Pressure (SBP) Diastolic Blood Pressure (DBP) และ proteinuria ได้มากกว่า manidipine ในขณะที่ manidipine ไม่ทำให้ proteinuria เปลี่ยนแปลง[12] อย่างไรก็ตาม สามารถเพิ่มยา manidipine ในผู้ป่วยความดันโลหิตสูงที่ควบคุมไม่ได้และเป็น microalbuminuria ที่ได้รับ full dose ของยากลุ่ม ACEIs ได้เนื่องจากมีผลลดอัตรา urinary albumin excretion และความดันโลหิต[13]
โดยสรุป ยา manidipine มีความปลอดภัยและมีประโยชน์ต่อไตมากกว่ายา amlodipine โดยสามารถลด intraglomerular pressure และการขับ urinary albumin[3]อย่างไรก็ตามหากผู้ป่วยมีภาวะ proteinuria ควรใช้ยาในกลุ่ม ACEIs[12] เป็นยาอันดับแรกและหากมีการปรับยา กลุ่ม ACEIs ในขนาดเต็มที่แล้วและยังควบคุมความดันโลหิตไม่ได้สามารถเพิ่มยา manidipine เพื่อเสริมการรักษาได้[13]
