Topical Antihistamine สามารถช่วยลดอาการคัน และลดอาการแพ้ได้หรือไม่

เนื่องจากทราบมาว่า ไม่สามารถบอกได้ว่า Topical antihistamine สามารถลดอาการคันได้อย่างมีนัยสำคัญ และไม่สามารถหา Pharmacokinetic ของ Topical antihistamine ได้ จึงอยากทราบว่าจริงๆแล้ว Topical Antihistamine ที่เห็นตามท้องตลาด มีประสิทธิภาพหรือไม่

Topical antihistamine แบ่งออกเป็น 2 ประเภท คือ (1) topical antihistamine for skin และ (2) ophthalmic topical antihistamine
...
(1) Topical antihistamine for skin กลุ่มยานี้ ใช้บรรเทาอาการคัน ผื่นแดง หรืออาการแพ้ต่าง ๆบริเวณผิวหนัง ซึ่งส่วนใหญ่มีข้อมูลการศึกษา และแนะนำแนวทางเฉพาะกรณี localized pruritus ดังนี้
• Doxapin 5% cream : ยานี้ แม้จะเป็นเป็นยากลุ่ม tricyclic antidepressant แต่มีฤทธิ์ต้าน histamine H1 และ H2 receptors โดยมีหลักฐานคุณภาพดีสนับสนุนจาก randomized controlled trials (RCTs) ว่าสามารถลดอาการคันได้อย่างมีนัยสำคัญทางสถิติในผู้ป่วย atopic dermatitis และ localized pruritus [2-3]
• Dimetindene maleate gel : เป็น H1 antagonist ที่มีการศึกษาในกลุ่มผู้ป่วยเด็กติดเชื้อ varicella zoster พบว่า สามารถลดอาการคันได้เมื่อเทียบกับยาหลอก [4] อย่างไรก็ตาม ข้อมูลสนับสนุนยังมีจำกัด และไม่ได้เป็นยาที่แนะนำใน guideline ทั่วไป
• สำหรับตัวยาสำคัญอื่น ๆ ที่มีในท้องตลาด เช่น diphenhydramine, chlorphenoxamine ยังไม่พบหลักฐานคุณภาพสูงสนับสนุนประสิทธิภาพในการบรรเทาอาการคัน นอกจากนี้ ยังมีการศึกษาในหนูทดลอง พบว่า topical antihistamines เช่น diphenhydramine, doxepin และอื่น ๆ อาจมีฤทธิ์ลดการอักเสบและช่วยซ่อมแซม skin barrier [5] อย่างไรก็ตาม ยังไม่สามารถสรุปประโยชน์ทางคลินิกจากข้อมูลนี้ได้

(2) Ophthalmic topical antihistamine ใช้รักษาอาการคันตาจากโรค allergic conjunctivitis โดยกลไกผ่านการยับยั้ง histamine H1 receptor และ mast cell stabilization จากแนวทางการรักษา allergic conjunctivitis [6] และหลักฐานจาก RCTs พบว่ายาต่อไปนี้มีประสิทธิภาพดีในการลดอาการคันตาเมื่อเทียบกับ placebo
• Olopatadine, alcaftadine, bepotastine, ketotifen, epinastine, emedastine : มีหลักฐานสนับสนุนชัดเจนในด้านการลด ocular itching โดยเฉพาะใน seasonal allergic conjunctivitis [7-10]
• Naphazoline : เป็น vasoconstrictor มากกว่าฤทธิ์ antihistamine ไม่แนะนำให้ใช้เป็นยาเดี่ยวในการรักษาอาการคันตา และไม่มีข้อมูลสนับสนุนประสิทธิภาพที่เพียงพอ [11]
• Azelastine : มีข้อมูลบางส่วนจากการศึกษาขนาดเล็กถึง moderate effect ในการลดอาการคัน [12]
