ผู้ป่วยได้รับยา Alfacalcidol ชนิดรับประทาน ต้องการเปลี่ยนเป็นยา calcitriol ชนิดรับประทาน เนื่องจากปัญหาการเบิกจ่าย อยากทราบว่ายา 2 ตัวนี้มี equivalent dose กันหรือไม่

Calcitriol (1α,25-dihydroxycholecalciferol หรือ 1α,25(OH)2D) เป็นรูปที่ออกฤทธิ์ (active metabolite) ของ vitamin D มีมวลโมเลกุล 416.6 g/mol[6] ส่วน Alfacalcidol (1-α-hydroxycholecalciferol) เป็นอนุพันธ์ของ calcitriol ที่ได้จากการสังเคราะห์ มีมวลโมเลกุล 400.6 g/mol[4] ซึ่งเมื่อมีการ hydroxylation ที่ carbon ตำแหน่งที่ 25 ในตับ จะได้รูปที่ออกฤทธิ์ คือ calcitriol[1,2]

alfacalcidol มีฤทธิ์เท่าเทียมกับ calcitriol ในการกระตุ้นการดูดซึม calcium ของลำไส้และการสะสมแร่ธาตุของกระดูก (bone mineralization)[2] alfacalcidol และ calcitriol มีข้อบ่งใช้ที่เหมือนกันในการรักษาภาวะ hypocalcemia, secondary hyperparathyroidism, osteodystrophy ในผู้ป่วยไตวายเรื้อรัง และใช้รักษาโรคกระดูกอ่อนที่เกิดจากการดื้อวิตามินดี (vitamin D resistant rickets), hypophosphataemic vitamin D resistant rickets[3-6] ขนาดยา alfacalcidol และ calcitriol รูปแบบรับประทานที่มีการจำหน่ายในท้องตลาด ได้แก่ 0.25 mcg และ 0.5 mcg[3,5,7,8] มีชื่อทางการค้าดังไฟล์แนบ

ในการออกฤทธิ์ alfacalcidol จะเกิดการ hydroxylation ที่ตับเป็นรูปที่ออกฤทธิ์ (active metabolite) คือ calcitriol และจากการคำนวณ mol ของยา alfacalcidol และ calcitriol จากขนาดยา 0.25 mcg และ 0.5 mcg ที่ใช้ในการออกฤทธิ์ แล้วเปรียบเทียบกัน พบว่า mol alfacalcidol : mol calcitriol เท่ากับ 1.04 : 1 ซึ่งมีค่าใกล้เคียงกัน ดังนั้น ยา alfacalcidol และ calcitriol สามารถใช้แทนกันได้ที่ขนาดยาเท่ากัน จึงสามารถสรุปได้ว่ายา alfacalcidol และ calcitriol ชนิดรับประทาน มี equivalent dose กัน โดยมีขนาดยาที่จำหน่ายในท้องตลาด คือ 0.25 mcg และ 0.5 mcg
